There’s a completely different, well-documented reason someone can eat a perfectly reasonable diet and still develop a real nutrient shortfall: the medications sitting in their cabinet.

This isn’t a fringe theory. It’s a recognized clinical phenomenon called drug-induced nutrient depletion, and it affects an enormous number of people who have no reason to suspect their prescription is the actual source of their fatigue, brain fog, or muscle aches.

The Diabetes Drug That Quietly Drains B12

Metformin is one of the most widely prescribed medications in the world, and it’s also one of the best-documented examples of this phenomenon. Multiple peer-reviewed studies have found that long-term metformin use is associated with reduced vitamin B12 levels, with reported prevalence of deficiency among long-term users ranging widely across studies — commonly cited figures span roughly 6% to 30%, and some study populations have shown rates considerably higher depending on age, duration of use, and how deficiency was defined. Research on this overlap specifically warns that B12-deficiency-related neuropathy can be mistaken for diabetic neuropathy, meaning the actual cause can go unaddressed for years while both patient and doctor reasonably assume the diabetes itself is to blame.

The Heartburn Medication and a 2011 FDA Warning

A second, independently documented example involves one of the most commonly used classes of medication in the world: proton pump inhibitors, prescribed for acid reflux and ulcers. In 2011, the FDA issued a formal drug safety communication after reviewing dozens of adverse event reports, warning that long-term PPI use — generally after three months or more, with most cases emerging after a full year — was associated with low magnesium levels, in some cases severe enough to cause seizures, abnormal heart rhythms, or muscle spasms. The FDA noted that roughly a quarter of the cases it reviewed required stopping the medication entirely, since magnesium supplementation alone wasn’t enough to correct the problem while the drug was still being taken.

The suspected mechanism, as later research described it, involves reduced intestinal absorption of magnesium — the same category of problem seen with metformin and B12, where the issue isn’t dietary intake but the body’s ability to actually take up what’s being consumed. The FDA’s own communication acknowledged that hypomagnesemia from this cause is likely under-recognized, since low magnesium produces vague, easily-dismissed symptoms — fatigue, muscle cramps, anxiety-like symptoms — long before it becomes severe enough to be obviously connected to the medication.

Why This Category of Problem Is Genuinely Easy to Miss

What connects these two well-documented cases is a specific blind spot in how people evaluate their own health. When someone eats reasonably and still feels depleted, the natural next step is scrutinizing diet more closely — when the actual mechanism may have nothing to do with what’s on the plate at all, and everything to do with a pill taken every morning for an entirely unrelated condition. Nutrient depletion from a long-term medication doesn’t announce itself as medication-related. It shows up as generic fatigue, brain fog, or muscle weakness — symptoms so common and nonspecific that they’re easy to attribute to stress, aging, or “just needing better food,” when the actual fix might have nothing to do with diet at all.

This is compounded by the fact that many of the medications involved are taken for chronic conditions over years or decades, precisely the timeframe over which these depletions tend to develop — slowly enough that there’s no obvious moment connecting a new symptom to an old prescription.

What This Actually Calls For

None of this is a case for stopping any medication without medical guidance — metformin and PPIs are effective, important treatments for real conditions, and the goal here isn’t alarm, it’s awareness. If you’re on a long-term medication and experiencing fatigue, cognitive fog, muscle symptoms, or other vague complaints that “adding more vegetables” hasn’t touched, it’s a reasonable, evidence-backed step to ask a healthcare provider whether your specific medication has documented nutrient interactions — and whether targeted bloodwork (B12 and methylmalonic acid for metformin users, serum magnesium for long-term PPI users) makes sense given how long you’ve been on it.

The broader lesson is one this whole subject keeps circling back to: “eating enough” was never actually a guarantee of being well-nourished. Sometimes the gap isn’t in what you’re eating at all — it’s in what’s quietly, systematically interfering with your body’s ability to use it.
